CLIENTS
​ELIGIBILITY​
The individual must be twenty-two (22) years old or older and eligible to receive ID waiver services through the community services board. The individual may have behavioral challenges, but not actively hostile to others.​
​STEPS FOR ADMISSION
Schedule time to meet with the director to meet the individual and discuss your needs and desires as it relates to residential services and day support.
Contact your current Case Manager and select Life Line Residential on the Consumer Choice form.​

​Upon acceptance, a collaborative effort will occur which will include: the individual, the case manager, residential service providers, Day Support , and anyone that the individual would like to contribute to their plan such as a family member or authorized Representative (AR).

The Individual Service Plan is developed to meet the needs and interest of the individual.
The individual and the guardian are given the choice to accept or deny the services made available at all times.
A preliminary Individual Service Plan and ISAR must be submitted by the Case Manager and approved by DBHDS staff.
